Ah, yes, the "all-white" issue... note there's only 5 and a half pages of white panels..., but that's all it takes for a comic to get a reputation as being completely white. I still love this issue, every single milky white blank page. Thanks for the post! White

Not sure, but there was an earlier version in What If...? v1. #34. Which also had an Alpha cameo! (panel? appearance? What ever #35.)

Not absolutely sure of the when, but I'm aware of an issue of Daredevil, where a lot of the panels where done from his perspective. I'mpretty sure it was before AF number 6.

Not the answer to the trivia question, but wow, bizarre timing for a new book just published yesterday. In the FCBD 2009 Avengers book from Marvel, there's a Frost Giant who shoots a blizzard at the Avengerii (plural of Avengers, I just made it up) and yeah, there are at least 2 panels of complete white. Ha! Another all-white completely blank comic book! Of course the "writer", Brian Michael Bendis, who sucks and who I totally hate, doesn't have any talent and just left the panels without any text or thought bubbles or narration.
Oh, and the What If...? issue mentioned above was #34, yes it has an Alpha Flight cameo, but here is the white panel:
